Certified Diabetes Outpatient Educators - trained Registered Nurses, Registered Dietitians, or Pharmacists - lead individual or group consultations in doctors' offices, pharmacies or dedicated sites.
Who we help
Adults (18+ years old) with diabetes referred by a healthcare provider.

Covered under some health insurance plans, may require co-pay.

- Evaluate your current balance and suggest resources to meet your balance needs
How this works
Participate in a series of physical tests and receive recommendations from a licensed physical therapist on how to improve your balance and reduce falls. Physical tests are completed by a licensed rehabilitation therapist or doctorate physical therapist student.

- Teach you how to eat a balanced diet based on the USDA's MyPlate framework
- Improve heart health through diet
How this works
Teaches you how nutrition and lifestyle changes can promote better health. The program focuses on heart and bone healthy nutrition strategies to help maintain or improve wellness and prevent chronic disease development or progression. Healthy Eating uses the USDA’s MyPlate as a framework.

- Caregivers communicate more effectively in challenging situations, recognize the messages in their emotions, deal with difficult feelings, and make tough caregiving decisions.
- Caregivers to develop a wealth of self-care tools to reduce personal stress, change negative self-talk, and communicate their needs to family members and healthcare or service providers.
How this works
Facilitated by two certified peer leaders in group sessions of 1 1/2 hours per week for six weeks. Class participants will receive a copy of The Caregiver Help Book, developed specifically for the class.

How this works
This no-cost program can help you achieve your healthier lifestyle goals. The program follows a national, evidence-based curriculum approved by the Rhode Island Department of Health (RIDOH) and Centers for Disease Control and Prevention (CDC). Group sessions meet once a week for 16 weeks. Continuing support is offered twice a month, then monthly. Learn more about Ready For Health program.
