Tuberculosis Information For Providers

Case reporting

  • Active tuberculosis cases are required to be reported within four days of recognition. Complete and return an Active Tuberculosis Case Report Form to the Tuberculosis Program.
  • Latent Tuberculosis Infection (LTBI) cases are required to be reported within four days of recognition. Complete and return a Latent Tuberculosis Case Report Form to the Tuberculosis Program.
