Find Inspections, Violations & Disciplinary Actions

The Rhode Island Department of Health conducts inspections at licensed facilities throughout Rhode Island. In some instances, inspections at facilities are routine. In other instances, inspections at facilities are in response to complaints. 

Additionally, the Rhode Island Department of Health conducts investigations in response to complaints about licensed professionals. 

While inspection reports and disciplinary actions are available publicly, complaints are confidential. The confidentiality of the complaint process is necessary to help ensure that patients, health professionals, customers, and all members of the public bring health and safety concerns to the attention of the Rhode Island Department of Health. 

Disciplinary actions and orders are posted here. The information posted includes the date the action or order was entered and the full text of the decision. Information about matters still under investigation is not available. Additionally, actions taken before December 09, 2014 may not be accessible through this system. Public information regarding these unposted actions can be obtained by submitting a records request.

Inspection Reports and completed Complaint Investigation Reports are currently posted for hospitals, nursing homes, and assisted living facilities. Inspection Reports and Complaint Investigation Reports for other licensed healthcare facility types are in the process of being made available online.

Drinking Water Viewer provides access to information from the state’s database for public drinking water systems. This includes sample results, inventories, sampling points, schedules, violations, and enforcement actions. Additionally, RIDOH posts all health advisories for public drinking water systems.
