Immunization Records
An immunization record is an official document that lists the vaccines a person has gotten throughout their lifetime. It proves a person is protected against preventable diseases, avoids unnecessary repeat vaccinations, and helps healthcare providers identify which vaccines may be due. Keeping an accurate, up-to-date immunization record is essential to maintain personal health and meet school or work requirements.
For people born in 1997 or later
The Rhode Island Department of Health (RIDOH) started collecting immunization information in 1997 using KIDSNET, Rhode Island’s secure database for children’s critical healthcare data. In October 2020, this registry expanded to include adult immunization information using the Rhode Island Child and Adult Immunization Registry, or RICAIR.
How to request a copy of your immunization record
RIDOH does not provide copies of immunization records to members of the public. Immunization records can only be released to healthcare institutions, childcare facilities, schools, colleges, or employee occupational health offices. Members of the public should contact their healthcare provider, childcare facility, school, college, or employer to request a copy of your immunization record.
For help sending records to healthcare institutions or school or college health services, contact RIDOH.RICAIR@health.ri.gov or access and submit the RICAIR Immunization Record Release Form (Portuguese) (Spanish).
Medical and religious exemptions for school entry are processed by the school that your child attends. Please contact your child’s school nurse for help.
For people born before 1997
RICAIR has very limited immunization information from the years before 1997.
The best source for vaccination records is your primary care office, but they may not have records from your childhood.
Primary care healthcare professionals have access to RICAIR and may have additional information from other sources. Many primary care offices have patient portals where you can access your immunization record.
Other resources that may have your immunization record
The Rhode Island Medical Society may have information about where records from a deceased or retired healthcare professional are stored. Gorwood Medical Systems and Iron Mountain are the most common record storage locations.
