Community-Driven Outreach for Hypertension Management and Diabetes Prevention – FY2026 – Short-Term Project Funding Opportunity

The Rhode Island Department of Health’s (RIDOH) Diabetes and Cardiovascular Health Program (RIDCH) invites applications from eligible organizations and agencies to apply for a Short-Term Funding Opportunity to create and implement products or events that aim to promote hypertension management and diabetes prevention in Providence communities.  RIDCH’s intention is to award a minimum of ten (10) organizations up to $4,999 per award.  Eligible applicants must serve Providence communities and be affiliated with, or a part of, a not-for-profit, community-based agency, coalition, and/or grassroots organization that has a Federal Employer Identification Number (FEIN) or a Federal Tax Identification Number.  For-profit entities are not eligible.

Applications should be submitted by no later than 4:00 p.m. on Friday, March 20, 2026 to: Gina DeLuca at Gina.DeLuca@health.ri.gov .  The deadline for the submission of questions is 4:00 p.m. on Wednesday, March 11, 2026.  Questions should be submitted in writing to Gina DeLuca at Gina.DeLuca@health.ri.gov with “Short-Term Project Opportunity” in the subject line.  At the conclusion of the Q & A period, all questions and official answers will be posted as an addendum on the RIDOH website.  RIDOH will not be responding directly to applicants.
