Chronic Conditions

Chronic conditions are conditions that last a long time and require ongoing care. They include arthritis, asthma, diabetes, heart disease, special needs (such as autism) and diseases (such as cancer). The symptoms of chronic conditions sometimes come and go or flare up at certain times. Even when the symptoms come and go, there are some things that can be done to help manage the chronic condition.
