What we do

Bring the perspective of parents, youth, and consumers into policy development and medical home implementation.

Connect consumers with community resources such as specialty care, independent living, education, employment, and vocational training.

Provide Peer/Family Resource Specialists to both primary and specialty healthcare practices.
