Community Health Workers for COVID Response and Resilient Communities Grant Strategies

Since 2021, the RIDOH Community Health Worker (CHW) Program has administered the federally awarded Community Health Workers for COVID Response and Resilient Communities (CDC-RFA-DP21-2109) grant in Rhode Island. To accomplish this work, RIDOH has formed partnerships with community-based organizations and agencies located throughout Rhode Island. Priorities and focus areas of this work align with two federal CDC grant categories, known as Component B and Component C

Component B: These strategies and activities expand deployment of CHWs in the State of Rhode Island.

Component C: Focused activities and strategies address policy, system, or environmental change to strengthen the roles of CHWs for addressing health disparities and social inequities made worse COVID-19. Such CHW interventions can improve uptake and access to healthcare services, improve communication between community members and healthcare providers, reduce the need for emergency and specialty care services, and improve adherence to providers’ health recommendations.

Grant-Funded Initiatives

Community Health Workers for COVID Response and Resilient Communities

In 2022, RIDOH’s Division of Community, Health, and Equity sought competitive proposals and awarded contracts for 15 agencies and organizations to implement CCR-Grant strategies and activities to train, deploy, and engage Community Health Workers (CHWs) toward COVID-19 response and recovery efforts and toward building and strengthening community resilience to fight COVID-19 through addressing existing health disparities. The RIDOH CHW Program solicited and conducted a competitive review of Request for Proposals (RFPs) to award approximately $1.79 million in combined funding among 11 community partner organizations. Awardees include:

  • Open Doors 
  • Parent Support Network of Rhode Island 
  • Providence Community Health Centers 
  • Providence Housing Authority 
  • Refugee Dream Center 
  • South County Health

  • Thrive Behavioral Health
