To provide home health services to include nursing, physical therapy, occupational therapy, speech therapy, medical social work, home health aide, home maker in-home support, and needed supplies and equipment. The service areas are, but not limited to: Providence, Kent and Bristol counties. All services, programs, policies and procedures will be developed with the support of qualified consultants with legal, home health, hospice, infusion, and health care program development and accreditation experience with culturally sensitive care for all with the inclusion of the Armenian under served population.

How to get licensed

Establishment and Certificate of Need approval of a Home Care Provider requires prior review by the Health Services Council and written approval of the Director of Health. Completion and submission of the Letter of Intent form and the Certificate of Need Application is a prerequisite to licensure. The Letter of Intent form must be submitted prior to submission of the Certificate of Need application to the Center for Health Systems Policy and Regulation.

Change in owner, operator, or lessee of a licensed Home Care Provider requires prior review by the Health Services Council and approval by the licensing agency. Completion and submission of the Change in Effective Control application is a prerequisite to any change. The Change in Effective Control application shall be submitted to the Center for Health Systems Policy Regulation.more
