Chronic conditions are conditions which last a long time and require ongoing care. Chronic conditions is a broad term which encompasses both special healthcare needs, such as autism, and diseases, such as cancer. The symptoms of chronic conditions sometimes come and go or flare up at certain times. Even if the symptoms come and go, there are some things that can be done to help manage the chronic condition.
The Division of Community, Family Health, and Equity within the Rhode Island Department of Health aims to achieve health equity for all through eliminating health disparities, assuring healthy child development, preventing and controlling disease and disability, and working to make the environment healthy.
